Total Bile Acids (TBA) – Clinical Significance

Total bile acids reflect how well the liver, biliary system, and enterohepatic circulation are functioning. They are synthesized in the liver from cholesterol and normally recycled efficiently between the intestine and liver.

Why the test matters

Sensitive marker of liver dysfunction – TBA often rises earlier than ALT, AST, or bilirubin, especially in mild or early liver disease.

Assesses hepatic clearance – Elevated levels suggest impaired uptake, conjugation, or excretion of bile acids by hepatocytes.

Typical sensitivity range

Most modern enzymatic or LC-MS/MS based TBA assays have a sensitivity of around
0.5–1.0 µmol/L

Advanced LC-MS/MS methods can go even lower (≈ 0.1 µmol/L)
