Clinical Significance of Small Dense LDL-Cholesterol (sdLDL-C)

Small dense LDL (sdLDL) refers to a subclass of low-density lipoprotein (LDL) particles that are:

  • Smaller in size

  • Denser

  • More atherogenic than large, buoyant LDL

LDL particles vary in size and density; sdLDL represents the more harmful phenotype.

1️⃣ Highly Atherogenic

sdLDL particles:

  • Easily penetrate the arterial wall

  • Bind less efficiently to LDL receptors

  • Remain longer in circulation

  • Are more susceptible to oxidation

Oxidized LDL promotes:

  • Foam cell formation

  • Fatty streaks

  • Atherosclerotic plaque formation


2️⃣ Independent Cardiovascular Risk Factor

Elevated sdLDL is strongly associated with:

  • Coronary artery disease (CAD)

  • Myocardial infarction

  • Stroke

Even when total LDL-C is normal, high sdLDL increases risk.
