1. Assessment of Short-Term Glycemic Control

  • Reflects recent changes in blood glucose (2–3 weeks).
  • Useful when rapid evaluation of therapy effectiveness is required.
  • Examples:
  • After starting or changing insulin/oral hypoglycemics
  • During unstable or poorly controlled diabetes

2. Alternative Marker When HbA1c Is Unreliable

Fructosamine is preferred in conditions affecting red blood cells, such as:

  • Hemolytic anemia
  • Hemoglobinopathies
  • Recent blood loss or blood transfusion

Because it depends on serum proteins, not RBC lifespan, results remain reliable.

3. Monitoring Diabetes in Pregnancy

  • Glycemic control can change quickly during pregnancy.
  • Fructosamine allows frequent, short-interval monitoring compared with HbA1c.

4. Diabetes with Altered RBC Turnover

Useful in chronic kidney disease or conditions with shortened RBC survival where HbA1c may be falsely low.

5. Monitoring Rather Than Diagnosis

Best suited for follow-up and monitoring, not primary diagnosis of diabetes.
